Who is Eligible?
Children from birth to age 26 and pregnant women regardless of income, are served by CDS. CDS accepts all forms of insurance and offers reduced-cost care under an income-based sliding scale.
CDS offers care out of two clinical headquarters, located at 636 Broadway Street NE and 728 E Hennepin Ave in Northeast Minneapolis, and at over 600 satellite clinics within schools, Head Start centers and other community locations across Minnesota and North Dakota including in Duluth, Fargo/Moorhead, the Iron Range, and St. Cloud. CDS’ highly skilled staff is exceptionally well equipped to serve families from culturally diverse backgrounds. Most CDS staff members themselves are bi- or multilingual, together speaking numerous languages, including American Sign Language, Cambodian, Dingo, Farsi, French, Hindi, Hmong, Karen, Korean, Russian, Serbo-Croatian, Somali, Spanish, Swahili, Ukrainian, Urdu, and Vietnamese.
CDS has certified MNSure Navigators that can help assist you in applying for Minnesota Medical Assistance. Depending on eligibility, Navigators can help you apply either over the phone or in person at one of our two clinics, Monday-Friday.
If you are not eligible for Medical Assistance, CDS offers an income-based, sliding fee scale for income eligible families. To apply for sliding scale with CDS, we ask for the following documentation:
Rejection letter from Minnesota Medical Assistance
Most recent income tax forms (1040 or 1040A)
Pay stubs for the last two months for all working members in household.
